Please note our office reserves your appointment time especially for you therefore, we require  2 days notice for cancellations, at least one day notice to avoid a cancellation fee.

Payment Policy

Payments are due on date of service, If you have dental insurance – we will request your deductible and co-pay at time of service, our office can estimate  coverage from your insurance, but recommend  that  patients have their insurance booklet for details since we do not have access to all coverage information.  (we can only give estimate of coverage)    

Any balance that remains from estimate will be billed, payments being due within 20 days of billing to avoid late fee.

Insurance:  We take all PPO and Indemnity (regular) dental insurance.  We are a provider of most PPO insurances.

Our office takes Visa, Mastercard, checks, cash, and CareCredit
